DUO FOB Request - Multi-Factor Authentication (MFA)

Service Description

DUO FOBs allow for use of multi-factor authentication (MFA), sometimes called two-factor authentication, which requires the use of a physical device in addition to a password when logging into an account. Without both factors – the device and the password – you cannot log in. This makes a stolen password alone effectively worthless.

Any active Mines' student, faculty, or staff member.

A mobile device or designated FOB device is required for this service.

Service Charges

This service is provided free of charge. However, standard data rates from your carrier may apply if using your mobile device off campus (not on Wifi).

Requesting the Service

If you want to enroll in Duo using your phone or tablet as the primary device, please checkout our self-service enrollment portal at https://mfa.mines.edu/. (You need to be on campus or connected through VPN – reference the Knowledge Base article here.) Self-service is not available for student employee accounts. In this case you must proceed with the service request on this page.

Support can also be requested by visiting the Mines Service Center (MSC) or calling x2345 or 303-384-2345 during MSC staffed times. In this case, a Student Consultant will take your information.

This document will quickly walk-through the self-service enrollment process for MFA (Multi-Factor Authentication) available at https://mfa.mines.edu/. The vendor being used to provide this service to Mines is Duo Security.
Multi-factor authentication (MFA) makes accounts more secure by requiring approval via a second method (generally a mobile phone) to log into particular, critical services. DUO is the system by which Mines allows for multi-factor authentication. Use this Request Service to order new activations, FOB keys, or hardware tokens. To report lost / stolen devices or if you need a temporary bypass, please use the MFA Issues Form.
