What is it?
This service is used to notify the Controller’s Office of a request for disposal of capital equipment or capital software.

Capital assets are defined as tangible equipment or software, costing more than $5,000 and in service for greater than one year. Capital assets are tagged with an Asset ID number (typically with a sticker) and are depreciated and inventoried annually.
When capital equipment or software is no longer in service, a disposal form must be completed to remove the equipment from inventory. The Department Head and Controller’s Office will approve the disposal.
After this form is completely signed, remove Mines tag sticker from equipment and contact:
