Completion of these forms is required for Mines employees to be granted access to various parts of the Banner Admin pages. Students do not need to complete any of these forms unless they become student workers. In addition to the forms below, employees may need to complete other forms (such as confidentiality agreements, and so on) as required by various Mines departments.
