3.0 Instructions
Follow these steps to duplicate a Microsoft Form and share it as a template:
- Navigate to https://forms.microsoft.com

- Locate the form you want to duplicate and select Open.

- Click the ellipsis (three dots) in the upper-right corner and select Collaborate or Duplicate.

- Under Share as a template, click the Copy button.

- Paste the copied link into your browser. On the page that opens, click Duplicate this.

- The form will now appear under your personal account.
