Routing and Approval Process for ALL Academic Faculty Forms/Changes

Adjunct Faculty

  • All new and renewal appointments are done through PageUp
    • If adding additional duties to a current adjunct, complete the Payroll Action Form found in the MAPS Form Directory. and submit as noted above.

Research Faculty

  • All new and renewal appointments are done through PageUp
  • Any index or FTE changes need to go on the Payroll Action Form found in the MAPS Form Directory.  Changes need to be signed by the fund manager (and supervisor if the fund manager is the same person) and submitted as noted above.
  • Any supplemental pay, professional consulting or extra pay for extra services needs to go on the Payroll Action Form found in the MAPS Form Directory.  Any additional pay for research faculty needs to be submitted to pcesforms@mines.edu

Academic Faculty

  • All new appointments are done through PageUp.
  • Any supplemental pay, professional consulting or extra pay for extra services needs to go on the Payroll Action Form found in the MAPS Form Directory.  Any additional pay for academic faculty needs to be submitted to pcesforms@mines.edu

Winter Research for Academic Faculty

  • The Winter Research Payroll Action Form, found in the MAPS Form Directory, will need to be completed for any academic faculty charging an index for winter pay.  This will require the following signatures:Employee, DH and the Fund Manager (if different than DH).Once all signatures are obtained, submit as noted above.

Administrative & Classified Faculty/Staff

  • All new appointments are done through PageUp.
  • Any supplemental pay, professional consulting or extra pay for extra services needs to utilize the new Payroll Action Form found in the MAPS Form Directory. Any additional pay for academic faculty needs to be submitted to pcesforms@mines.edu

Notes

  • For any professional consulting or extra services for extra pay, the approval forms (located on AA’s website) for such services have to be completed, submitted and signed PRIOR to submitting PAF’s for payment. 
  • All extra services for extra pay forms MUST be submitted to Jennie Kenney in AA for Provost’s approval and signature.  Approval forms not submitted PRIOR to the work being completed will NOT be paid.
  • All Payroll Action Forms (PAF) are still on the HR website.  In the next several months, they will be moving to a Payroll site (perhaps MAPS).  At that time, Payroll will be looking at archiving the hrforms@mines.edu emails address and using a new one.

PageUp – Contractor Module

Several questions have come up as to when to use the contractor module in PageUp.  Any hire with an end date should be in the contractor module.  This way, it is easy to extend the appointment when and if it is needed.

Several PIs or Fund Managers (FM) and signatures

Note that if you have several different PIs or FMs that need to sign the PAFs, you can add them to the spreadsheet by increasing the size of the box to fit them all in.

Charge-Outs

Currently charge-outs are done on the Academic HR Form.  Budget has taken this on themselves to adjust the form for their needs.  This form is posted on the HR Website.
